The Physical Therapist will provide care to patients in a home-visit setting and must be comfortable traveling between patient locations throughout the day.
Holiday coverage may be required as needed. The facility will conduct its own background check.

Requirements
- Current Physical Therapist license required
- Must have OASIS experience
- Must have Home Health experience
- Must have Hospice experience
- Must have experience traveling from patient to patient for home visits
- Must be able to manage and adjust to changes in the schedule
- EMR: HealthWyse
- Holiday coverage may be required as needed
- Facility conducts its own background check
- Contract: 13 weeks, 32 hours/week, Monday-Friday 8:00 AM-5:00 PM
