Assignment details:

- Contract length: 13 weeks

The Physical Therapist will provide care to patients in a home-visit setting and must be comfortable traveling between patient locations throughout the day.

Holiday coverage may be required as needed. The facility will conduct its own background check.

Requirements

  • Current Physical Therapist license required
  • Must have OASIS experience
  • Must have Home Health experience
  • Must have Hospice experience
  • Must have experience traveling from patient to patient for home visits
  • Must be able to manage and adjust to changes in the schedule
  • EMR: HealthWyse
  • Holiday coverage may be required as needed
  • Facility conducts its own background check
  • Contract: 13 weeks, 32 hours/week, Monday-Friday 8:00 AM-5:00 PM
